Emergency Care Card (online form)

This one form may NOT be signed digitally, per Archdiocesan policy.

Please fill out the information online and when you attend Meet & Greet, we will have this form printed for you to sign. If you are unable to attend Meet & Greet on August 5th, please complete the form online and come into school on the morning of August 6th, and we will have the form ready for you to sign at the front desk.
